Claims do not always move in a straight line.

A rejection can lead to a corrected submission. Adjudication can include denial or adjustments. Preserve the earlier attempt and response when another step follows.

Read the sender, not just the label.

Clearinghouse and payer acknowledgments can describe different points in the route. A 277CA acknowledgment is different from an 835 remittance. Neither replaces reconciliation of a bank receipt.
